RESULTADOS POSTOPERATORIOS EN LESIONES DEL MANGUITO ROTADOR / Postoperative results in rotator cuff tears

RESUMEN

El manguito rotador es un componente integral en el normal movimiento y función del hombro, tanto estática como dinámicamente. La enfermedad del manguito rotador constituye una de las principales causas de dolor y disfunción a este nivel en los adultos, repercutiendo de forma directa sobre la productividad laboral. La valoración del paciente que presenta dolor en el hombro debe ser de forma esquemática e integral, abarcando datos epidemiológicos, laborales, anamnesis, examen físico y estudios complementarios que incluyan radiología, ecografía y resonancia magnética. En el servicio como protocolo a todo paciente con diagnóstico de lesión o no del manguito rotador se le aplica la escala de UCLA, sirviendo como un indicador clínico sobre la evolución y pronóstico pre y postoperatorio. El objetivo de esta investigación fue evaluar los resultados postoperatorios en el tratamiento de lesiones del manguito rotador en el servicio de Ortopedia y Traumatología del Hospital Adolfo Pons, Maracaibo, Venezuela. Materiales y métodos: fue un estudio retrospectivo, longitudinal, descriptivo, observacional, durante el periodo julio 2013 a junio 2017. Se seleccionaron 60 pacientes con diagnóstico de lesión del manguito rotador operados mediante técnica miniopen, con edades comprendidas de 40 a 60 años de ambos sexos. Se aplicó formato de recolección de datos y escala de UCLA preoperatorio y postoperatorio. Análisis de los resultados con SPSS 23.0. Entre los resultados, se pudo observar que el sexo femenino predominó con un 73.3%. Un El 76.6% presentó acromion tipo III. El 50% de los casos presentó infiltración grasa por resonancia magnética Estadio 2. El 76.7% presentó ruptura parcial del manguito rotador. La media de puntaje UCLA postoperatorio fue de 29 puntos. En conclusión, las lesiones del manguito rotador son más frecuentes en pacientes femeninas, amas de casa con acromion tipo III, con mayor incidencia de rupturas parciales aisladas

ABSTRACT

The rotator cuff is an integral component in the normal movement and function of the shoulder, both statically and dynamically. Rotator cuff disease is one of the main causes of pain and dysfunction at this level in adults, having a direct impact on labor productivity. The assessment of the patient with a painful shoulder must be schematic and comprehensive, including epidemiological, occupational, anamnesis, physical examination and complementary studies that include radiology, ultrasound and magnetic resonance; In our service as a protocol to all patients diagnosed with a rotator cuff injury or not, the UCLA scale is applied, serving as a clinical indicator on the evolution and pre- and postoperative prognosis.Objective: to evaluate the postoperative results in the treatment of rotator cuff teras in the Orthopedics and Traumatology service of the Hospital Adolfo Pons, Maracaibo, Venezuela. Materials and methods: retrospective, longitudinal, descriptive, observational study, during the period July 2013 to June 2017. Sixty patients with diagnosis of rotator cuff tears operated by mini open technique, with ages ranging from 40 to 60 years of both sexes, were selected. Data collection format and UCLA scale were applied preoperatively and postoperatively. Analysis of the results with SPSS 23.0.Results: the female sex predominated with 73.3%. A 76.6% presented acromion type III. 50% of the cases presented fatty infiltration by magnetic resonance imaging Stage 2. 76.7% presented partial rupture of the rotator cuff. The mean post-operative UCLA score was 29 points.Conclusions: Rotator cuff tears are more frequent in female patients than housewives with acromion type III, with higher incidence of isolated partial ruptures.
